A suggested Military Service Affidavit is provided in the Local Rules/Forms page on this website.  A Department of Defense Military Service Certificate concerning a defaulting person’s military service status may be obtained at: A default judgment cannot be entered against a party without the filing of the Military Service Affidavit.  This link may be helpful in verifying military service status.

A Subpoena form is available on this website in PDF format.  Four signed copies must be submitted to the Court in person or by mail for issuance and return to the Applicant.  (see Local Rules/Forms page on this website).
