
You may request
an Absentee Ballot in writing or vote in person at the Board office.
This same service applies to voters in the Armed Services, who will
be away from home during the day of election.

to your computer (in PDF format). Just fill out the form, and mail
it back to the address listed on the form, and your information will
be updated and your voting eligibility confirmed.

To request an Absentee Ballot, use the request form or write us a note. Your note must include the following required information;
1.A statement that you are a Qualified Elector in the County
2. Your name
3. Voting residence address
4. The mailing address to send the Absentee Ballot
5. Birthdate
6. The Election for which the ballot is requested
7. Phone number
8. Your signature
9. If this is for a primary election, also indicate what type of ballot you are requesting (Republican, Democratic, other party, or Issues Only)
10. And one of the following: your Ohio drivers license number, the last four digits of your social security number, a copy of your current and valid photo identification, a military identification, or a current (within the last 12 months) utility bill, bank statement, government check, paycheck or other goverment document (other than voter registration notification mailed by the board of elections) that shows your name and address.
